How do I create a form where only the areas that need to be completed are editable.

the document can be used for both single entry and mail merge

You can follow the steps here

but I would start with a blank document, rather than any template available in Word.

Basically if you have the Developer tab enabled, enter Design Mode and then insert the controls that you want.

When you are finished, click on Restrict Editing (under Developer tab) and then set editing restrictions to only allow filling in forms.

I've created a sample form in the attached doc.  The password to disable editing restrictions is abc

You can't use a form for a mail merge.  Once you tell Word that you want to use the doc for a mail merge it removes the ability to restrict editing.
