Below Please see small workbook attached , I need help in creating table properly , so Column 1, Column 2, Column 3, Column 4, Column 5 does not
appear. I created so many tables in past, but, I am having difficult time creating this table properly.

The problem is your data is not consistent.
When you make a table every column has it's own kind or value and every row got the proper information.
I made a little start with it based on your data.
When you complete this new table and make it an Excel table, you can use it as source for a pivottable.
In that pivot table you can present the data in the way you made your table.
